Getting around
You can bus the whole Cape on the Cape Cod Regional Transit, and ferries link Hyannis and Provincetown to Boston. In summer the rail trail and town shuttles work, but a car (or a bike plus patience) makes the Outer Cape far easier. Traffic on Route 28 on summer weekends is legendary, so plan midweek drives.
Safety
Very safe overall. The real hazards are ocean rip currents on Atlantic-side beaches, ticks in dune grass (check yourself after hikes), and sun exposure on the flats. Swim at lifeguarded beaches and respect posted shark advisories, which are common on the Outer Cape.
